You must then turn in the application to Families First School Readiness in one of three ways: 1) mail it to 126 Woodlake DR SE, Rochester, MN, 55904, 2) drop it off at the front desk during business hours on Monday through Friday from 8 a.m. to 4:30 p.m., 3) drop it in the red drop box outside on the northwest corner of the Families First of Minnesota building, open even after business hours.

To PICK-UP an application, please stop by the School Readiness office at: Families First of Minnesota, 126 Woodlake DR SE, Rochester, MN.
With every application, we also require Proof of Income. Proof of income is required to be considered for enrollment. Please provide one of the following:
- Copy of 2022 Federal Tax Return
- Copies of all 2022 W-2’s
- Copies of last three pay stubs for each employed adult in household
